Annual Report 2023-24

From our Leadership

The spotlight often shone brightly on primary care throughout 2023-24, highlighting its vital importance and challenges.

As a Primary Care Network (PCN), we know the value of primary care and continuously strive to support the family doctors that make up our membership, patients, and our team members.

Our 2023-24 Annual Report provides an overview of many, but not all, of the ways we worked across our four priority areas in the last fiscal year to provide this support, including two prominent projects to support our members with quality improvement (QI) and to engage stakeholders in new program development:

  • To support our members with QI and practice improvement, our PCN developed and shared support guides and action plan examples with members, organized online and in-person educational events that provided practical guidance, and offered facilitated consultation sessions with our QI expert. These supports and services help members identify and engage in QI work with the goal of improving patient care.
  • To help our development of a new Dietitian Program and a new Physiotherapy Program, we consulted with many different individuals and organizations, ranging from our members to patient representatives, our clinical teams, and a host of AHS- and community-based partners.

We appreciate everyone — our members, patients, teams, and stakeholders — who engaged with our PCN in the last fiscal year; please read on to learn more about our PCN in 2023-24.

We are a non-profit organization — the Calgary West Central Primary Care Network (CWC PCN) — committed to a healthier future. Throughout our annual report, we’ll share how we worked to serve our community and built on our previous efforts.

You will find our major accomplishments of the 2023-24 fiscal year broken into four priority areas:

Highlights from 2023-24

Mental Health Program evolution:Improving access with evening and weekend appointments.
Boosting access:Our clinic, the Primary Care Centre, expanded weekend and holiday hours while still serving the same record-setting number of referrals as in 2022-23.
Developing new programs:Broad stakeholder engagement played a vital role in developing our new Dietitian Program and Physiotherapy Program.
Supporting QI needs:Our PCN launched a support project to assist doctors with the College of Physicians & Surgeons’ requirements for practice improvement.
Keeping safety top-of-mind:Our commitment to physical and mental safety once again earned recognition from independent parties.
Better together:We collaborated with four service providers to make their 24-hour blood pressure monitoring more accessible and convenient for patients.
